Intensive Case Management brings together a team of dedicated case managers offering a wide range of services across the Halton Region to those with complex and polysubstance concerns.
Our Mission
Our mission is to foster hope and provide comprehensive, person-centered, holistic services to those affected by substance use.
We understand that people begin using substances for many reasons and that, for some, this may begin to feel unmanageable as time goes by.
Our Services
- Brief individual counselling and support, this includes skills such as; safety planning, relapse prevention & coping skills
- System navigation and collaborative care across community supports
- Help to access additional treatment and support services
- Education and support to cope with chronic pain
- Support with stress and mental health issues
- Information about Methadone and Suboxone treatment, referrals to OAT clinics
- Education and support for family members and friends concerned about a loved one’s substance use
- Help with related issues such as: finances/income, legal issues, work/school, & finding a family doctor
- Referrals to in-patient treatment
- Collaborates with community partners through case conferences and consultation for the purposes of coordinated case management and client care, substance use and substance dependence education

Our team can arrange to meet you in a safe location of your choice in the community, at our offices or virtually via phone/zoom.
Our services are designed from a harm-reduction perspective which focuses on enhancing your health and well-being by preventing or reducing substance-related harms. Our team works with a range of goals from abstinence to harm reduction and those in between.
